Filled out Scholarship Application Form (4 pages inclusive of Statement of Purpose)

Proof of income ( a copy of any of these documents):
- The latest income tax return form of your parent or guardian
- If relevant, your most recent income tax return form
- A letter or budget sheet indicating the total benefits you receive from the state (such as AFDC, Social Security, VA, and others)
- A statement affirming you had no income due to imprisonment. This statement needs to be provided by an authority or representative of the institution where you were incarcerated.
- A signed statement indicating that someone else provided for you. The income details of this person are also necessary.
A transcript from your present or last attended high school, accredited college, or vocational school within the past two years
1 recommendation letter from an individual acquainted with your academic or professional work. (Letters from relatives and friends are not allowed)
Statement of Purpose (an essay not exceeding 250 typewritten words)

PROCEDURE FOR SUBMITTING YOUR SCHOLARSHIP APPLICATION

Digital Submission:

The Scholarship application can be completed online
Save it on your desktop or laptop
Attach the filled-out PDF Application in an email to be sent to scholarship@conntacinc.org
Supporting documents should be scanned and attached to the email as well.
All digital submissions should be sent on or before the due date specified on the scholarship application (Page 1 of 4)

U.S. citizen residing in Connecticut who is pursuing post-secondary education and/or vocational training
U.S. citizen residing in Connecticut who showcases financial, academic, cultural, or social need
Note: Eligibility is evaluated by the Counselor at CONNTAC-Educational Opportunity Center
